AWARD GUIDELINES

Complete the nomination form and submit with the following background materials:

Letter describing achievements and contributions of nominee.

Identify why the nominee meets criteria for that specific award with an emphasis on measurable outcomes of the nominee’s work. Letter must be typed, double spaced and not exceed two pages.

Up to three letters of support from individuals other than the nominator are optional but strongly recommended.

These letters are not to exceed two pages and should address the nominee’s qualifications for the award.

Current resume, curriculum vitae and/or narrative description
of nominee.

Document must be typed.

All materials must be submitted with the nomination form as the cover document.

Please review the guidelines before submitting materials to ensure all guidelines are met.

Please endorse (printed name and signature) nomination form before submitting materials.

Nomination forms without a signature will not be considered.
