Award Categories

Advocacy/Public Policy Award

Criteria: Awarded to public official or advocate for outstanding leadership in advancing one of HWTF’s public policy priorities and for their efforts in advancing issues related to preventive health in North Carolina.

Community Catalyst Award

Criteria: Recognizes an individual or an organization that has established a community-based project and/or coalitions that have significantly improved the health and well-being of the citizens of a community or generally improved the status of community health and disease prevention in NC.

Innovator Award

Criteria: Awarded to an individual or organization that is recognized for designing and delivering a new and innovative approach to health care and disease prevention in NC.

Leadership in Obesity, Tobacco Use Prevention / Cessation or Health Disparities

Criteria: Individual recognized for pioneering work in raising awareness and/or providing direct services to NC communities related to obesity, tobacco use prevention / cessation or health disparities.

Deadline: Nomination forms and corresponding materials must be received (not postmarked) by 5 p.m. Friday, March 28, 2008.
