AWARD GUIDELINES

Complete the nomination form and submit with the following background materials:

Letter describing achievements and contributions of nominee.

Identify why the nominee meets criteria for that specific award with an emphasis on outcomes of the nominee’s work. Letter must be typed, double spaced and not exceed 2 pages.

Letters of support are optional, but strongly recommended.

These letters are not to exceed 2 pages and should address the nominee’s qualifications for the award. We suggest no more than 3 letters.

Current resume, curriculum vitae and/or narrative description
of nominee.

Document must be typed.

All materials must be submitted with this nomination form as
cover sheet.

Please review the guidelines before submitting materials to ensure all guidelines are met.

Please endorse (printed name and signature) application before submitting materials.

Nomination forms without a signature will not be considered.
